So I hit level 50 I clicked on the level 50 quest it didn't give me the memory wipe potion so I disband it and now it's not showing up for me

Check your inventory again, it’s a black crystal with a yellow border. If you have it then you will have to discard it before the quest will show up again. (If you provide a screen shot of your bag I will gladly look and see if I can find it as well)
