Frenzied Destroyer Double Attack VI - Broken?

On my main Berserker if I activate the last rank of Frenzied Destroyer (VI) I can no longer do the Double Attack. (Yes - I know the combo is tricky but I do it 100% of the time before the upgrade):

Any idea what is going on? I tested removing all skill addons, and rebooting the PS4.

Well, this particular bug is either only on my character or on PS4. My guild mates are able to do it at max rank.
